Personal Support Worker (PSW)
Position Summary:
As a Personal Support Worker in our Long-Term Care facility, you will provide direct, hands-on care to residents, supporting their daily living activities while promoting dignity, independence, and overall quality of life.
​
Key Responsibilities:
-
Provide companionship, engage in social activities, and encourage resident participation.
-
Maintain a clean, safe, and comfortable environment.
-
Support safe mobility and transfers using assistive devices as needed.
-
Assist with meals including feeding and monitoring dietary restrictions or preferences.
-
Assist residents with personal hygiene, grooming, dressing, bathing, and toileting.
-
Observe and report changes in residents’ physical or emotional conditions to nursing staff.
-
Comply with infection control protocols, health and safety standards, and LTC regulations.
-
Document care provided and resident observations accurately and promptly.​
​
Qualifications:
-
PSW Certificate from an accredited program (NACC-accredited preferred).
-
Experience in long-term care or similar settings is an asset.
-
Compassionate, patient, and respectful with strong interpersonal skills.
-
Ability to work collaboratively as part of a healthcare team.
-
Ability to communicate in Chinese (Mandarin or Cantonese) is strongly preferred.

Registered Practical Nurse (RPN)
Position Summary:
As an RPN in our Long-Term Care facility, you will be responsible for delivering nursing care and coordinating with the healthcare team to support the health, safety, and well-being of residents.

Key Responsibilities:

Supervise and support PSWs and other support staff in the delivery of care.
-
Communicate effectively with residents, families, and other healthcare professionals.
-
Provide direct nursing care including medication administration, wound care, and chronic disease management.
-
Monitor, assess, and document residents’ health status and respond to changes promptly.
-
Develop and implement individualized care plans in collaboration with the multidisciplinary team.
-
Ensure compliance with LTC home policies, professional standards, and Ministry of Health guidelines.
-
Participate in quality improvement initiatives and staff training programs.

Qualifications:
-
Current registration with the College of Nurses of Ontario (CNO).
-
Diploma in Practical Nursing from an accredited institution.
-
Experience in long-term care or geriatric nursing preferred.

Excellent clinical, organizational, and leadership skills.
-
Ability to work independently and in a team environment.
-
Ability to communicate in Chinese (Mandarin or Cantonese) is preferred.
